Upper Kuldahar is, presumably, an additional district of Kuldahar.

The party cannot travel there in the Icewind Dale game or any of it's expansions.
In fact, the only reference to the existence of upper Kuldahar is this dialog line by Whitcomb (the barkeeper of Kuldahar), talking about Lysan (one of his barmaids):
